Your Initial Appointment
When arriving for your initial office visit, please arrive fifteen (15) minutes prior to the scheduled appointment time, so that your chart may be completed prior to being seen by the doctor. Please be sure the following information is available prior to arriving:

Subsequent Appointments
Please make us aware if there are any changes in your family doctor, insurance company, or current medications.
Cancellations
If you must reschedule or cancel an appointment, we ask that you give us 24 hours notice so we may give that time to another patient. Please see Cancellation Policy (PDF).
Co-pays must be paid at the time services are rendered. We accept cash, check, Visa, MasterCard and Discover. All deductible amounts, coinsurance and balances after insurance has paid are due upon next visit or first billing statement.
